Flu Shot Clinic October 15th
We are having a flu clinic at Ruby on Friday, October 15th from 1-2pm for students and staff. Information will be given to parents of students as well so that they may have their child immunized at this clinic.
- Please bring your insurance card to the clinic.
2. Complete the vaccine consent form. You can use the below link to fill-out the consent online, print it and bring it along with you to the clinic. You may also pick up a copy of the consent in the office.
(Flu Shot Consent forms will be sent home with students. Please check your child’s backpack).
