Free School Physicals offered by DMU (Do you need one for Special Olympics?)

Des Moines University will offer free back-to-school physicals for students on Thursday, July 21st and Friday, July 22nd.

Walk in on Thursday the 21st from Noon to 5:00 p.m. or Friday from Noon to 4:00 p.m.

If you will be using this physical so that your child will be eligible for the Special Olympics, please be sure to tell the receptionist when you check in so that they can use the correct form.  You can print the Special Olympics Athlete Medical Form from THIS LINK , pick one up at the Ruby Van Meter office or ask the Des Moines University staff for a copy of the form when you arrive.
